May include communicating with health care teams, setting up services (housekeeping, meals, transportation etc), form completion and more.
Caregiver Respite/ Companion–
Set up weekly caregiver respite by offering meaningful companionship to give the main care giver a break.
Transition Consultation–
Tour of various facilities or meeting to go over different facility options based on needs. Assistance signing up for the waiting lists.
Basic Referral or Form Completion-
Referring senior to various resources or funding available. Completing applications for various forms.
Living Independently: Your Guide to Aging in Place
This session will guide you through essential planning tips to maintain independence and remain in your own home as long as possible. Learn about important considerations like enhancing home safety, accessing resources, funding, caring for a spouse, advanced care planning and more so you can enjoy your life at home for years to come.
